Agreement shall be available for inspection by any person in accordance <br />with Chapter 119, Florida Statutes. The DAY CAMP PROVIDER shall comply <br />with Federal and State law to ensure the confidentiality of personal <br />information including name, Social Security Number, Social Security <br />benefit data, date of birth, official State or government issued <br />driver's license or identification number, alien registration number, <br />government passport number, employer or taxpayer identification number, <br />home address, phone number and medical information of each Camper <br />receiving a COUNTY Day Camp Scholarship Voucher. <br />(b) The DAY CAMP PROVIDER shall have in place an official process <br />for investigating and reporting lost or stolen confidential information. <br />Any infraction or potential infraction of State and/or Federal laws <br />related to the loss or theft of confidential information shall <br />immediately be reported to the COUNT .:jn writing. <br />(c) The parties hereto shall retain all financial records, <br />supporting documents, and any other documents pertinent to this <br />Agreement for a period of five (5) years after the termination or <br />expiration date, whichever is earlier, or such longer period as may be <br />required by Federal or State law. <br />Section 6. Notices. All notices, consents, approvals and required <br />reports which any party shall be required or shall desire to make or <br />give pursuant to this Agreement, shall be given in writing and <br />delivered to the below designated persons by First Class United States <br />mail or facsimile transmission, proof of delivery and receipt retained <br />by the sending party: <br />Seminole County / City of Casselberry <br />Day Camp Provider Agreement <br />Page 7 of 13 <br />
