CASSCLBCRRY CITY COMMISSION <br />Minutes of October 14,2019 — Regular Meeting <br />Page 7 of 20 <br />L Task Authorization No. 1 with Killebrew,_Inc. for Live Oaks Force Main <br />Replacement Project <br />Introduction: Task Authorization No. 1 with Killebrew, Inc. in an amount not to exceed $90,698.73 <br />provides for the Live Oaks Force Main Replacement Project. <br />Budget Impact: Upon approval of Budget Amendment #20-005, funds in the amount of $150,000 will be <br />available in the Renewal and Replacement Distribution & Collection Infrastructure account #402-0751- <br />536.63-40. <br />Recommendation: The City Manager and the Public Works Director recommended approval of Task <br />Authorization No. I with Killebrew, Inc. for the Live Oaks Force Main Replacement project in the amount <br />of $90,698.73. <br />Task Authorization No. 3 with Southeastern Surveying and Mapping Corporation <br />for Surveying and Utility Services Related to the Windward Square Water Main <br />Replacement Project <br />Introduction: Approval of Task Authorization No. 3 with Southeastern Surveying and Mapping <br />Corporation, in an amount not to exceed $60,797, for professional surveying and utility services related to <br />the Windward Square Water Main Replacement Project is requested. <br />Budget Impact: Upon approval of Budget Amendment #20-005, funding in the amount of $60,797 will <br />be available in the Renewal and Replacement Distribution & Collection Water Improvements account <br />#402-0751-536.63-11. <br />Recommendation: The City Manager and the Public Works Director recommended approval of Task <br />Authorization No. 3 with Southeastern Surveying and Mapping Corporation for Windward Square Water <br />Main Replacement Surveying and Utility Services in the amount of $60,797. <br />K. Final Change Order No. 3 to the Agreement with Killebrew, Inc. for the Forest <br />Brook Force Main Replacement Project and Related Budget Amendment #20-006. <br />Introduction: Final Change Order No. 3 to the agreement with Killebrew, Inc. and related Budget <br />Amendment #20-006 provide for overruns related to the construction of the Forest Brook Force Main <br />Replacement Project in the amount of $4,438.60. <br />Budget Impact: Budget Amendment #20-006 increases the Water and Sewer Fund Other Non -Operating <br />Source Account #401-0000-389.90-00 and the Water and Sewer Distribution & Collection Infrastructure <br />Account #401-0751-536.63-40 by $4,438.60. <br />BA #20-006 <br />
