the location of the proposed installation of facilities in the rights-of-way <br />shall be to scale and show: <br />1. The offset from the centerline of the rights-of-way or <br />road to the proposed utility installation; <br />2. The road rights-of-way and pavement width; <br />3. The distance from the edge of the pavement to the <br />facility or utility; <br />4. Within a minimum of 100 feet of work all above <br />ground infrastructure and improvements, including without <br />limitation, pavement, curb, sidewalks, buildings, utility <br />poles, street furniture, landscaping} etc.; and all below <br />ground infrastructure and utilities, including without <br />limitation, foundations, tanks, utilities, etc. within limits of <br />work; <br />5. The location of all other utilities and facilities, <br />including wireless communication facilities, within the area <br />of work; <br />6. The facilities to be installed and their size; <br />7. Identification of the parcels consisting of the nearest <br />adjacent property and the zoning and jurisdiction of such <br />property; <br />8. One or more typical cross sections as required to <br />adequately reflect the location of the facility. The minimum <br />vertical clearance above or below the parkway or pavement <br />shall be shown. <br />9. Additional information, such as the location in <br />relation to the nearest town, major road intersections, <br />bridges, railroad crossings and other physical features, shall <br />be indicated on the drawings and identified. <br />b. A simple key map showing the location of the proposed <br />facility shall be included, either on the drawing itself or as a separate sketch, <br />to assist all concerned with the general location of the installation, <br />indicating the applicable section, township and range. <br />C. Applicable stormwater analysis, traffic impact analysis, and <br />maintenance of traffic plan for any disruption of the public rights-of-way. <br />d. Information on the ability of the public rights-of-way to <br />accommodate the proposed facility. <br />e. If appropriate, given the facility proposed, an estimate of the <br />cost of restoration to the public rights-of-way; <br />f. The means and methods in which the facilities shall be <br />installed. <br />g. For all facilities located within applicable proximity to an <br />intersection, a sightline obstruction analysis signed and by a Professional <br />Engineer licensed in the State of Florida, <br />Page 5 of 52 <br />
