CASSELBERRY CITY COMMISSION <br />Minutes of November 9, 2015 — Regular Meeting <br />Page 4 of 19 <br />BA #16 -013 <br />Explanation of Request: This item seeks City Commission approval to provide funding for additional <br />construction engineering and inspection (CEI) services, as outlined in Amendment No. 1 to Supplemental <br />Agreement No. 3 with CDM Smith, related to the South Triplet Lake Drive Improvements Project, <br />Amendment No. 1 to Supplemental Agreement No. 3 provides for the extension of CEI services through the <br />end of construction, as various issues during the construction phase of the project have resulted in a work effort <br />that will exceed CDM Smith's original proposed scope. Resolution 15 -2797 - Supplemental Agreement #4 to Extend the Term of the <br />Standard Consultant Agreement for Professional Services for Transportation <br />Engineering with CDM Smith, Inc. <br />"A RESOLUTION OF THE CITY COMMISSION OF THE CITY OF CASSELBERRY, FLORIDA, <br />APPROVING AND AUTHORIZING THE EXECUTION OF SUPPLEMENTAL AGREEMENT NO. <br />4 TO EXTEND THE STANDARD CONSULTANT AGREEMENT WITH CDM SMITH, INC. FOR A <br />TWELVE MONTH PERIOD; PROVIDING FOR CONFLICTS, SEVERABILITY, AND EFFECTIVE <br />DATE." <br />Introduction: Resolution 15 -2797 approves Supplemental Agreement No. 4 extending the Standard <br />Consultant Agreement for Professional Transportation Engineering Services with CDM Smith, Inc. for a <br />period of 12 months and authorizes the Mayor to execute said document on behalf of the City. <br />Budget Impact: There is no impact to the City Budget, <br />Recommendation: The City Manager and the Public Works Director recommended approval of Resolution <br />15 -2797. <br />
