Resolution 15 -2738 - Award of Bid and Contract with Integrity Golf Company, LLC <br />Resolution 15 -2738 provides for the award of bid for RFQ 2015 -0004 and execution of a contract with Integrity Golf <br />Company, LLC as the City's golf course management services firm, contingent upon the purchase of the Casselberry Golf <br />Club from Casselberry Golf Company. <br />G. Resolution 15 -2739 - Early Learning Coalition of Seminole Day Camp Provider Contract <br />Resolution 15 -2739 provides for consideration of a Day Camp Provider Contract with the Early Learning Coalition of <br />Seminole. <br />H. Revision to Capital Lease Loan Pay Off of Fire Department Vehicles and Related Budget <br />Amendment #15 -043 <br />Consideration of additional principal, interest, and administrative fee due in the amount of $55,832 to pay off the Fire <br />Engine and Rescue Vehicle Leases held under Schedule 10 of the Master Lease Agreement No. 0592 with SunTrust <br />Equipment Finance & Leasing Corporation and approval of related Budget Amendment #15 -043 are requested. <br />I. Piggyback Purchase of Emergency Medical Service Response Cart <br />Consideration and approval to purchase a multi- faceted Emergency Medical Service cart for event/trail coverage and <br />response in the amount of $16,985.94 by piggybacking the Florida Sheriffs Association Bid Award # 14 -12 -0904, <br />Specification #2, are requested. <br />J. Change Order No. 1 to the Layne Inliner Piggyback Contract for Gravity Sewer Lining for <br />Elm Drive, Wilshire Drive and Marigold Road Areas and Related Budget Amendment #15- <br />044 <br />Change Order No. 1 to the Layne Inliner piggyback contract for gravity sewer lining and related Budget Amendment #15- <br />044 provide for additional gravity sewer lining work in the Elm Drive, Wilshire Drive and Marigold Road areas in the <br />amount of $33,612. <br />K. Change Order No. 2 to the Layne Inliner Piggyback Contract for Gravity Sewer Lining for <br />the Elm Drive, Wilshire Drive and Marigold Road Areas and Related Budget Amendment <br />#15 -045 <br />Change Order No. 2 to the Layne Inliner Piggyback Contract for Gravity Sewer Lining and related Budget Amendment <br />#15 -045 provide for a deductive amount of $35,392 to re- adjust the construction project budget as appropriate for the <br />gravity sewer lining work done in the Elm Drive, Wilshire Drive and Marigold Road areas. <br />L. Change Order No. 1 to Supplement No. 1 of the Continuing Professional Services Agreement <br />with Ardaman & Associates, Inc. for Phase II Environmental Assessment on the Casselberry <br />Golf Club Property <br />Change Order No. 1 to Supplement No. 1 of the Continuing Professional Services Agreement with Ardaman & <br />Associates provides for additional work in conducting a Phase Il environmental assessment on the Casselberry Golf Club <br />property in an amount not to exceed $15,130.50. <br />M. Reorganization of the Human Resources and Risk Management Division <br />This item provides for consideration to reorganize the Human Resources Division in the Administration Department by <br />reclassifying the position of Health and Safety Coordinator to a position of Human Resources Specialist and by <br />reclassifying the position of Human Resources Coordinator to a position of Human Resources Generalist. <br />7. PRESENTATIONS /COMMENDATIONS <br />A. Special Presentation - Best Workplaces for Commuters <br />Representatives from reThink and /or the Center for Urban Transportation Research will present the City with Best <br />Workplaces for Commuters designation. <br />8. OTHER ITEMS <br />A. Mayors' Challenge for Safer People, Safer Streets <br />This item requests approval and authorization for the City of Casselberry to join the Mayors' Challenge for Safe People, <br />Safer Streets and designate a team leader. <br />B. Board Appointments <br />Applications are being presented for consideration for appointments to the Planning & Zoning Commission, the Parks & <br />Recreation Advisory Board and the Lakes Management Advisory Board. <br />
