Recommendation: The City Manager and staff recommended approval of the Special Invoice Report, as submitted. B. Resolution 13 -2499 — Business Associate Agreement with Intermedix Corporation for Rescue Ambulance Billing and related Professional Services "A RESOLUTION OF THE CITY COMMISSION OF THE CITY OF CASSELBERRY, FLORIDA, APPROVING AND AUTHORIZING A BUSINESS ASSOCIATE AGREEMENT WITH INTERMEDIX CORPORATION TO COMPLY WITH THE FINAL HIPAA OMNIBUS RULE ISSUED JANUARY 17, 2013; TO INCORPORATE LANGUAGE PERTAINING TO PUBLIC RECORDS AND A HOLD HARMLESS AND INDEMNITY CLAUSE; PROVIDING FOR CONFLICTS, SEVERABILITY AND EFFECTIVE DATE." Introduction: Resolution 13 -2499 approves and authorizes the Business Associate Agreement with Intermedix Corporation for ambulance billing and related professional services and authorizes the Mayor to execute said document on behalf of the City of Casselberry. Budget Impact: There is no impact to the City Budget. Recommendation: The City Manager and the Finance Director recommended approval of Resolution 13- 2499. C. Resolution 13 -2505 — Amendment No. 2 to the Drinking Water State Revolving Fund Construction Loan Agreement (DW590301) with the Florida Department of Environmental Protection and Related Budget Amendment #13 -059. "A RESOLUTION OF THE CITY COMMISSION OF THE CITY OF CASSELBERRY, FLORIDA, APPROVING AND AUTHORIZING THE EXECUTION OF AMENDMENT NO.2 TO THE STATE REVOLVING FUND LOAN AGREEMENT (DW590301) WITH THE FLORIDA DEPARTMENT OF ENVIRONMENTAL PROTECTION; PROVIDING FOR CONFLICTS, SEVERABILITY, AND EFFECTIVE DATE."
