He recommended that the businesses should be required to <br />maintain records for a minimum of two years to allow for investigative work if a patient dies. <br />Mr. Parkhurst asked for a motion to address MISC 10-04. Mr. Aramendia made a motion to provide a <br />favorable recommendation to the City Commission for MISC 10-04 moratorium on City's approvals for cash only <br />pharmacies and pain management clinics within the city limits of Casselberry and to change the moratorium to a 365 <br />day moratorium. Property located at 1870 State Road 436, Winter Park, Florida. <br />Mr. Eric Raasch, Planner I, explained that this project received site plan approval as Monarch Plaza and the <br />plaza name has since been changed to Danubis Plaza. Mr. Raasch reviewed the information provided in his <br />memorandum dated December 3, 2010, supported by a PowerPoint presentation, to the City of Casselberry Planning <br />and Zoning Commission (a copy is on file in the Community Development Department). Mr. Raasch said if the <br />Planning and Zoning Commission chooses to recommend approval to the City Commission for SP 10-01 for the <br />Danubis Plaza preliminary re-plat of 1860 and 1870 State Road 436, Casselberry, Florida, the approval should be <br />based upon the re-plat received November 3, 2010, the findings of fact and conclusions of the staff report dated <br />December 3, 2010, and the following conditions: <br />1. All required revisions to the plat shall be made and the City Surveyor shall execute the signature block <br />prior to referral of the final plat to the City Commission. <br />2. The final plat shall be submitted in the following form prior to review by the City Commission: (14)11" x <br />17" copies and (2) 20" x 24" copies. <br />3. The applicant, per F.S. Chapter 177, is responsible for the City Surveyor's fees. Prior to recording the <br />final plat, the applicant shall submit a check made payable to the "City of Casselberry" to cover the City <br />surveyor's fees. <br />
