C. The ENGINEER will prepare the Operations and Maintenance Performance
Report (OMPR) to include the following work:

1. Perform a site visit with City staff to inspect the current condition of the
wastewater facility.

2. Prepare an OMPR based on FDEP's Guidelines for Preparation of
Operation and Maintenance Performance Reports. The OMPR will
describe in detail each component of the facility and state the current
condition of each. The OMPR will include a statement identifying if the
facility is capable of providing proper treatment for the next five (5) years,
or recommending any necessary repairs and revisions to ensure it will
operate sufficiently for the next five (5) years.

D. The ENGINEER will review and revise the Operating Protocol, as necessary,
in accordance with FDEP requirements, which includes the following items:

1. Review procedures and criteria outlined in the current operating protocol
to ensure compliance with FDEP requirements and permit conditions
under both normal and adverse weather conditions.

2. Evaluate effectiveness of procedures and criteria noted above and
including assessing any violations of the permit requirements and
conditions of the existing operating permit.

3. Provide analysis and correlation of parameters sampled and test by the
laboratory to continuously monitored parameters such a turbidity and total
chlorine residual.

4. Revise Operating Protocol to include any proposed or necessary changes
to be reviewed and approved by the FDEP.

E. The ENGINEER shall prepare necessary information and documents related
to permitting supplemental water supplies to augment the City's existing
permitted public access reuse system during peak demand periods.
Descriptions of the proposed supplemental water supplies are as follows:

1. Ground water: The City proposes to equip and connect an existing eight
(8) inch City well to the City's public access reuse distribution system.
This well was a former potable water well.

2. Orinkinq water: The City proposes to extend a water main to the
reclaimed water storage tank to augment tank levels as needed. An air
gap will be provided equal to at least two times the diameter of the fill
pipe.

3
