<br />Mr. Ed Torres, M.S., P.E. <br />Page 3 <br />September 15, 2008 <br /> <br />Record shall perform up to three site visits during construction as well to answer any <br />questions regarding the construction documents'- <br /> <br />Task 5.0: Additional Services <br /> <br />This task will include any services that are not included in Tasks I through 4 and would <br />involve additional services beyond this scope of work. Such work includes additional data <br />collection, meetings or analysis and will be completed only upon the Client's direction. <br />Additional services will be completed on an hourly basis plus reimbursable expenses, <br />according to the attached rate schedule. <br /> <br />Assumptions: <br /> <br />. The survey provided by Southeastern Surveying and Mapping Corporation for the <br />design of the Winter Park Drive Extension will be used for the preparation of plans. <br />. The plans preparation will not be in accordance with Florida Department of <br />Transportation (FOOT) standards or the City of Casselberry standards. The plans <br />will be prepared in an emergency fashion utilizing existing survey, typical sections <br />for details and documentation sufficient fOr a contractor to bid the project as an <br />emergency. <br />. The City of Casselberry to provide plans for the Winter Park Drive Extension. <br />. A permit exemption for the proposed work will be obtained. <br />. The Consultant will not be providing full time construction inspection. <br /> <br />De'iverables <br /> <br />90% and Final Engineering plan submittals will include: <br />. Four (4) .hard copies of the construction plans for each submittal on II" x IT' <br />(construction plans to include a title sheet, one (I) general notes sheet, one (1) pay <br />items sheet, two (2) plan sheets. two (2) detail sheets, one (I) stonn water pollution <br />prevention plan sheet) <br />. One (I) hard copy of special specifications <br />. One (I) plan set in electronic PDF file format <br /> <br />Proiect Schedule & Fee <br /> <br />The anticipated schedule for this project is four (4) weeks for 90% plans submittal from the <br />Notice to proceed and then one (I) week for the Final Engineering Plans submittal once <br />written comments are received from the City. Post Design Services will be provided through <br />cOll1pletion of the project construction, which is anticipated to be one. hundred and twenty <br />(120) days from Notice to proceed. This schedule docs-not include and interim City review <br />time or permitting agency schedules. An estimated fee for this project is attached. <br /> <br />FEE <br />
