SECTION 1 <br /> <br />PURPOSE OFSUPPLEMENT <br /> <br />1.0 General <br /> <br />The purpose of this Supplement is to provide professional transportation engineering <br />services for the proposed intersection improvements at US Highway 17-92 and <br />Plumosa Avenue. The work will include Phase I analysis and Phase II design services <br />associated with the realignment of Plumosa Avenue and the Home Depot driveway at <br />US 17/92. The realignment will result in a single signalized intersection at US 17/92. <br /> <br />SECTION 2 <br /> <br />SCOPE OF PROFESSIONALTRANSPORTATION ENGINEERING <br /> RELATED SERVICES <br /> <br />2.0 General Services of the Engineer <br /> <br />The Phase I work effort will involve the preparation of a signal warrant analysis at the <br />intersection of US 17/92 and Plumosa Avenue. It is necessary to evaluate the <br />intersection for signalization (either existing or future), prior to design to determine the <br />feasibility of re-aligning the intersection. The fee for Phase I is (not to exceed) <br />$7~263,00, without prior written authorization. <br /> <br />The Phase II effort will involve the actual design, plan preparation, and permitting of the <br />intersection. To form a single intersection accommodating Plumosa Avenue and the <br />Home Depot driveway, Plumosa Avenue will have to be realigned approximately 50 feet <br />to the north, which will force the road to pass through the Concord Cemetery in the <br />northwest corner of Plumosa Ave. and US 17/92. There are 3 visible grave headstones <br />within the property but an adjacent property owner has indicated that there are several <br />unmarked graves. The Home Depot driveway will have to be shifted approximately 50 <br />feet to the south causing the realigned driveway to fall within what is now Lake Concord. <br /> <br />In order to construct the driveway in the lake, placement of fill will be required. To <br />minimize the amount of fill, TEl anticipates the use of retaining wall at the south end of <br />the realigned driveway. A St. Johns River Water Management District (SJRWMD) <br />Environmental Resource Permit and an US Army Corps of Engineers (USACOE) Permit <br />will be required for the impact to Lake Concord. In addition, a submerged land <br />easement may need to be prepared. It is likely that SJRWMD will consider this project <br />a reconstruction requiring provisions for storm water treatment.
