7c). Are you sharing an EIN# with your city and/or county? If so, please
name the parent entity. No.

7d). Do you have authorization to use this EIN#? Yes.

8a) Have you previously received a FEMA/DHS Assistance to Firefighters Grant?
Yes.

If you responded yes, please indicate what year(s) you were awarded, a brief
summary of what items were purchased, a current status on the awarded
grant(s) and their POP (Period of Performance) date.

In September 2004, the City of Casselberry was awarded EMW-2004-FG-14896-00l
in the amount of $499,500 for the purchase of a 75 ft. tower ladder truck and
for the development of training specific to the contracted apparatus. Funds
for this grant have been received in full with delivery of the truck and
final payment scheduled on September 15, 2005.

On August 12, 2005, the City's Grant Manager, Michele Toole requested an
extension of the training obligation section of this grant. Federal Emergency
Management Agency Grant Management Specialist Walter Pickett promptly
notified us that the Period of Performance and Article III of our Grant
Agreement Articles are amended to extend this grant from 9/17/04 through
12/16/05. We anticipate closure before 12/16/05.

* * * Closing out overdue AFG grants will facilitate further processing of
your FY 2005 application.

8b) Have you received any other federal funding this fiscal year? Yes.

If you responded yes, please indicate the name of the agency that awarded you
federal funding and a brief summary of the items that were awarded.

In fiscal year 2005 our City of Casselberry Police Department received from
the U.S. Department of Justice proceeds of $11,132 as a draw down of a law
enforcement Block Grant. The Police Department also anticipates $1,541.10
this fiscal year from the Department of Justice matching funds on the
purchase of bulletproof vests.

As a result of the approximate $3,000,000 hurricane storm damages incurred by
the City of Casselberry from August through September, 2004, the City has
received $1,388,694 in reimbursements recorded so far this fiscal year and is
expecting approximately another $1,000,000 with most of that probably coming
next fiscal year if not late this fiscal year.

The City's Public Works Department received $471,751 this fiscal year from
the U.S. Department of Environmental Protection (Pass-through to State
Revolving Loan Fund). This is the final draw down of a loan approved prior to
2005 for re-use water system expansion.

Again, this is not a notification of award¡ it is only a discussion of your
application. Once I complete my review, and receive your favorable reply to
this message, I will forward your application on to the next phase. If your
department is selected you will be notified bye-mail through our on-line
grants system. In that email, you will be directed to a website to print the
award forms and other pertinent information.

4
