GREEN UP CASSELBERRY GROWING COMMUNITY Exhibit A CITY OF CASSELBERRY ADOPT - A - SHORELINE APPLICATION Phone: (407) 262-7725 ext. 1229 ncooper@casselberry.org 95 Triplet Lake Drive, Casselberry, FL 32707 www.casselberry.org/lakes ADOPT — A — SHORELINE AGREEMENT TERMS AND CONDITIONS AND HOLD HARMLESS: I want to act as a Shoreline Sponsor in the City of Casselberry's shoreline planting program to help keep the lakes within the City of Casselberry healthy. The Public Works and Utilities Department will plant shoreline vegetation on my property. I hereby grant permission for the City and its contractors and representatives to access my property to perform the shoreline plant installation. I hereby hold harmless and release the City of Casselberry of all liability associated with the planting of the shoreline vegetation, any damage the City or its agents may cause during the shoreline planting, or any damage the shoreline vegetation may cause, or any damage the City causes to the shoreline vegetation. After the City plants the shoreline vegetation, I will regularly maintain and use care to protect the newly planted vegetation. If I move addresses, I will notify the City, and inform the person who resides at this address after me about this Agreement and ask them to assume shoreline care taking responsibilities. Shoreline planting is subject to available funding, availability of the selected plants, and suitability for my specific location.
