H. Budget Amendment 422-045 — Recognition of Revenue for Recreation Center Ballroom Mirror Introduction: Approval of Budget Amendment #22-045 to recognize a contribution in the amount of $2,000 from Seniors for a Better Community for the purchase and installation of a ballroom mirror at the Casselberry Recreation Center is requested. Budget Impact: Budget Amendment #22-045 will increase General Fund Contributions/Donations Private Sources Account #001-0000-366.91-00 in the amount of $2,000 recognizing a donation from Seniors for a Better Community to assist with the cost of replacing the ballroom mirror. BA #22-045 Explanation of Request: This item seeks City Commission approval to recognize the donation in the amount of $2,000 from Seniors for a Better Community to assist in the purchase and installation of the Casselberry Recreation Center ballroom mirror. The total cost of the mirror is $6,160. L Change Order No. 1 to Amendment No. 3 to Supplemental Agreement No. 2 with Wharton -Smith Inc. for a Guaranteed Maximum Price to Construct Lake Concord Park Art House Expansion and Related Budget Amendment #22-047 Introduction: Approval of Change Order No. 1 to Amendment No. 3 to Supplemental Agreement No. 2 to the Contract Agreement with Wharton -Smith, Inc. for Construction Manager at Risk (CMAR) for Parks Construction Projects for project PW 21 1 1 Lake Concord Park Art House Expansion and related Budget Amendment #22-047 in order to address existing stormwater infrastructure issues (Ref. SOQ-2021-0715) is requested. Budget Impact: Budget Amendment 422-047 will provide sufficient funding for this item by increasing Stormwater Fund Infrastructure Account 4110-0740-538.63-40 and Other Non-operating Sources Account #110-0000-389.90-00, each in the amount of $61,584.
