The purpose of this Supplement is to provide Mitigation Monitoring Compliance for Gee Creek, North <br />Winter Park Drive, Plumosa Avenue Intersection Improvements @ US 17/92 Intersection and Lake <br />Concord Stormwater Park as further described below: <br />1.0 PROJECT DESCRIPTION <br />1.1 Gee Creek <br />The SJRWMD issued Permit #40-117-116194-1 authorizing the modification and operation of a surface water <br />management system consisting of 0.392 acre erosion and sediment control measures along a 400 foot span of <br />Gee Creek known as Gee Creek Erosion Control (at Gee Creek Lane). The project extends 300 feet north of <br />Gee Creek Lane and 100 feet south of Gee Creek Lane and between Sundown Trail to the west and Osceola <br />Trail to the east. The City installed structural and non-structural erosion and sediment control measures <br />consisting of stacked gabions with plantings along the east side of the creek and riprap along the bottom of the <br />creek to its top of the bank. Project mitigation consisted of installation of restoration plantings into the pervious <br />banks of the project. The City planted approximately 5568 square feet (0.128 acre) of wetland and upland <br />trees and shrubs along the construction area. This proposal addresses the post -permit compliance monitoring <br />required in Permit Conditions No. 25. Services described below will be performed in a timely manner for <br />completion and submittal of the annual monitoring report to SJRWMD in August 2009. The specific scope of <br />services to be provided by CPH under this Agreement is provided in Exhibit 1. <br />1.2 North Winter Park Drive <br />The SJRWMD issued Permit #40-117-111941-1 authorizing the construction and operation of a surface water <br />management system for a 22.70 acre roadway development known as North Winter Park Drive from Seminola <br />Boulevard to State Road 434. The Mitigation associated with this project consists of preserving and enhancing <br />10 acres within a 29.81 acre city -owned parcel south of the city's water reclamation facility in Section 5 and 6, <br />Township 3N, Range 26E, Seminole County, Florida. Project mitigation consists of undesirable vegetation <br />removal and monitoring and maintenance for 5 years. This proposal addresses the post -permit compliance <br />monitoring pursuant to Permit Conditions No. 26. Services described below will be performed in a timely <br />manner for completion and submittal of the Baseline report to SJRWMD in August 2010. The specific scope of <br />services to be provided by CPH under this Agreement is provided in Exhibit 1. <br />1.3 Plumosa Avenue Intersection Improvements @ US 17/92 Intersection <br />The SJRWMD issued Permit #40-117-116055-1 authorizing the modification and operation of a surface water <br />management system for a 3.05 acre roadway improvement known as Plumosa Avenue at US 17/92 <br />Intersection Improvements. The Mitigation associated with this project consists of enhancing 0.38 acres of <br />riparian wetlands to a more endemic herbaceous marsh state on Lake Concord lakeshore. Project mitigation <br />consisted of undesirable vegetation removal and installation of restoration plantings. The City planted <br />approximately 0.38 acres of wetland and upland trees and herbs. This proposal addresses the post -permit <br />compliance monitoring and as -built survey required in Permit Condition No. 28. Services described below will <br />be performed in a timely mannerfor completion and submittal of the monitoring report to SJRWMD in August <br />2010. The specific scope of services to be provided by CPH under this Agreement is provided in Exhibit 1. <br />1.4 Lake Concord Stormwater Park <br />The SJRWMD issued Permit #40-117-29190-2 authorizing the modification and operation of a surface water <br />management system for a 9.28 acre governmental development known as City of Casselberry Lake Concord <br />Stormwater Park. The project is located at the northeast intersection of 17-92 and Melody Lane. The Mitigation <br />associated with this project consists of enhancing 0.75 acres of Lake Concord lakeshore. Project mitigation <br />consisted of undesirable vegetation removal and installation of restoration plantings. The City planted <br />approximately 0.75 acres of wetland and upland trees and herbs. This proposal addresses the post -permit <br />compliance monitoring and as -built survey required in Permit Conditions Nos. 22 and 26. Services described <br />Page 2 of I I <br />
