The 2011 through 2013 Annual Monitoring Reports documenting and summarizing the efforts and materials <br />employed to create the mitigation; and <br />The completed SJRWMD Annual Wetland Monitoring Report Form #EN 55. <br />Task 2.3 - North Winter Park Drive 2011 through 2014 Mitigation Monitoring and Annual Reports <br />Condition #26 —The Permittee must furnish the Districtwith two copies of an annual monitoring report <br />in the months of August 2011, August 2012, August 2013 and August 2014 on District form EN -55 for <br />the time period stated in the permit's success criteria condition. <br />The data collected for the annual monitoring reports shall include: <br />➢ Photographs from the 6 photostations - Each photograph station will have the corresponding location <br />on a map and the direction the photograph was taken. Three photostations will be established at <br />points along the edge of the restoration area and two along the landward edge and one along the <br />southern edge of the enhancement area; <br />➢ Percent cover and composition recording species composition for the mitigation area; and <br />➢ Recorded observations (general health, aesthetics, disturbances, wildlife use, etc.) of the mitigation <br />area to include as reference for the subsequent annual monitoring reports. <br />CPH shall prepare annual monitoring reports for submittal to the SJRWMD within 30 days after the completion <br />of the annual monitoring events. The annual monitoring reports shall include: <br />➢ Documentation of the methodologies CPH utilized to record data and observations; <br />➢ The report of results section documenting vegetative data, photostations and graphics; <br />➢ Maps showing the locations of the photographic stations, vegetative sampling stations, habitat types <br />and other features pertinent to the mitigation; <br />➢ The Annual Monitoring Reports documenting and summarizing the efforts and materials employed to <br />create the mitigation; and <br />v The completed SJRWMD Wetland Monitoring Report Form #EN 55. <br />Task 2.4 - Plumosa Avenue Intersection Improvements @ US 17/92 Intersection 2011 through 2014 <br />Mitigation Monitoring and Annual Monitoring Reports <br />Condition #28 — The Permittee must furnish the District with two copies of an annual monitoring report <br />in the months of August 2011, August 2012, August 2013 and August 2014 on District form EN -55 for <br />the time period stated in the permit's success criteria condition. <br />The data collected for the annual monitoring reports shall include: <br />➢ Photographs from the 2 photostations - Each photograph station will have the corresponding location <br />on a map and the direction the photograph was taken; <br />➢ Percent cover and composition recording species densities for the mitigation area; and <br />➢ Recorded observations (general health, aesthetics, disturbances, wildlife use, etc.) of the mitigation <br />area planting area to include as reference for the annual monitoring reports. <br />Page 9 of I I <br />
