SCOPE OF SERVICES <br />TASK 2.1 - Gee Creek 2001 SJRWMD Permit #40-117-1161941-1 Modification <br />CPH will prepare and submit a permit modification request to SJRWMD for Permit #40-117-1161941-1 that <br />shall include: <br />• Conducting one field inspection and meeting with SJRWMD staff regarding permit conditions and <br />success criteria; <br />• Conducting research to determine appropriate alternate mitigation within the Gee Creek project area <br />to facilitate the project success and monitoring requirements; <br />• Draft plans of the revised mitigation plan for submittal; <br />• Preparing a revised planting detail, if required; <br />• Submitting a revised success criteria condition for approval to the client prior to submittal to <br />SJRWMD; and <br />• Conducting a post -permit modification inspection with SJRWMD staff. <br />CPH will prepare and submit a permit modification request to SJRWMD for Permit #40-117-1161941-1 that <br />shall not include: <br />• Plant material and installation can be provided as approved by SJRWMD and as an additional <br />service. <br />Task 2.2 - Gee Creek 2011 through 2013 Mitigation Monitoring and Annual Reports <br />Condition #25 — The Permittee must furnish the district with two copies of an annual monitoring report <br />in the months of August 2011, August 2012 and August 2013 on District form EN -55 for the time period <br />stated in the permit's success criteria condition. <br />The data collected for the 2011 through 2013 Annual Monitoring Reports shall include: <br />➢ Photographs from the 4 photostations - Each photograph station will have the corresponding location <br />on a map and the direction the photograph was taken. <br />➢ Percent cover and composition shall record species densities for the mitigation area. <br />➢ CPH will record observations (general health, aesthetics, disturbances, wildlife use, etc.) of the entire <br />created wetland and restored upland to include in the annual monitoring report. <br />CPH shall prepare a report of results for submittal to the SJRWMD within 30 days after the completion of the <br />2011 through 2013 monitoring events. The annual monitoring report shall include: <br />Documentation of the methodologies CPH utilized to record data and observations; <br />The report of results section documenting vegetative data, photostations and graphics; <br />Maps showing the locations of the photographic stations, vegetative sampling stations, habitat types and other <br />features pertinent to the mitigation; <br />Page 8 of I I <br />
