City of Casselberry I Procurement and Contract Management Division 95 Triplet Lake Drive, Casselberry, Florida 32707 Phone: 407-262-7700, Ext. 1142 1 procurem ent(ab-casseIberry. org RFP -2020-0636 COMMERCIAL SOLID WASTE SERVICES AGREEMENT FOR OPEN -TOP ROLL -OFF CONTAINERS FOR NON-PUTRESCIBLE SOLID WASTE elects to instruct the Franchisee to service and remove the open -top roll -off container, the City shall be entitled to reimbursement from the commercial establishment served, plus all costs of collection, to include attorney's fees. 17.00 Reports (a) On the 25th day of the month following the month of service hereunder, the Franchisee shall provide the City Public Works Director with a report. Said report shall be in Microsoft Excel format, or otherwise as agreed satisfactory to the City. The report shall include such information as the City may reasonably require to ensure proper garbage service by all commercial establishments within the City and to ensure Franchisee's compliance with the terms and conditions of this Franchise. Unless otherwise directed by the City's Public Works Director, each report shall contain as a minimum: (1) Customer's Business Name. (2) Customer's Service Address. (3) Customer's Telephone Number. (4) Container(s) ID Number(s). (5) Container's Capacity (20 -cubic yard, 30 -cubic yard, etc.). (6) Pick-up Schedule for all Containers (or noted On -Call). (7) Number of Pick-ups for all Containers. (8) Invoiced Amount. (9) Franchise Fee. (10) Account Type (Non-putrescible Trash or Recycling). (b) The Franchisee must also provide the following reports; (1) Weight Reporting. The Franchisee must submit monthly to the City, a report showing all Solid Waste hauled under the Franchise . This schedule shall include: (i) total weight, and (ii) destination of the material (Landfill, Transfer Station, recycling processor) (2) Complaint Reporting. The Franchisee must submit monthly to the City: (i) a report of all misses, (ii) a report of all complaints, and (iii) a report of complaints not serviced within 24 hours (3) Service Data Reporting - On or before January 1 of each year, the Franchisee must provide to the City a current list of all vehicles/equipment. Failure to submit the reports required by this Section shall result in a $50.00 fine per day that the report is late.
