Inc., for the award of the construction of the Gee Creek Erosion Control Project Phase 11,111, & IV (Sheet Piling + Gabions) in an amount not to exceed $550,008.00. E. Change Order No. 3 to the Agreement with Hubbard Construction Company for the Road Rehabilitation -Various Streets Project PW 1604/ITB 2017-0127) and Related Budget Amendment #17-051 Introduction: Consideration to approve Change Order No. 3 to the Agreement with Hubbard Construction Company for the Road Rehabilitation -Various Streets Project (Project PW 1604/ITB 2017-0127) in the amount of $126,182.39 and related Budget Amendment #17-051 is requested. Budget Impact: Approval of Budget Amendment #17-051 will increase both the Infrastructure Surtax Fund/Intrastructure account#114-0710-541.63-40 and the Other Non -Operating Sources account#114-0000- 3 89.90-00 for $114,153. BA #17-051 Explanation of Request: This item seeks City Commission approval to re -appropriate funding to cover costs associated with Change Order #3 to the Road Rehabilitation — Various Streets Project with Hubbard Construction Company, in the amount of $126,182.39. The Public Works Department previously budgeted $104,145 for contingency for unanticipated field changes. Change Order # 1 was for an amount of $83,084.05; Change Order # 2 was for $9,030.80, leaving a remaining balance of $12,030.15 for contingency. Therefore, an additional $114,152.24 is required to complete the scope of work outlined in Change Order # 3. Appointment of the Human Resources and Risk Management Director Introduction: Consideration to ratify the City Manager's appointment of Andy Brooks as Human Resources and Risk Management Director effective October 1, 2017 is requested. Budget Impact: There is no impact to the City Budget. Recommendation: The City Manager recommended that the City Commission ratify the appointment of Andy Brooks as Human Resources and Risk Management Director, effective October 1, 2017.
