e. The location of all other utilities and <br />facilities, including wireless communication <br />facilities within the area of work⢠<br />f. The facilities to be installed and their Size" <br />g. Identification of the parcels consisting of the <br />nearest adjacent property and _ the zoning and <br />jurisdiction of such property: <br />h. One or more typical cross sections as <br />required to adequately reflect the location of the <br />facility. The minimum vertical clearance above or <br />below the parkway or pavement shall be shown. <br />L Additional information such as the location <br />in relation to the nearest town, major road <br />intersections, bridges, railroad crossings and other <br />physical features, shall be indicated on the drawings <br />and identified. <br />Ji. A simple key map showing the location of the <br />proposed facility shall be included, either on the drawing itself or <br />as a separate sketch, to assist all concerned with the general <br />location of the installation, indicating the applicable section, <br />township and range. <br />iii. Applicable stormwater analysis, traffic impact <br />analysis, and maintenance of traffic plan for any disruption of the <br />public rights -of --way. <br />iv. Information on the ability of the public rights-of- <br />way to accommodate the proposed facility. <br />v. If appropriate, given the facility proposed, an <br />estimate of the cost of restoration to the public rights-of-way; <br />vi. The means and methods in which the facilities shall <br />be installed. <br />vii. For all facilities located within applicable proximity <br />to an intersection, a sightline obstruction analysis signed and by a <br />Professional Engineer licensed in the State of Florida, <br />viii. For utilization of electric utility poles and facilities, <br />documentation of compliance with the requirements of the electric <br />utility's franchise agreement regarding facilities for other, non- <br />electric utility services or products. <br />ix. The timetable for construction of the project or each <br />phase thereof, and the areas of the City which will be affected. <br />(f) Upon approval of the application and payment of the fee, one copy <br />of the application, with attachments and the permit, will be returned to the <br />applicant, with a City approval stamp to be used as a field plan set. <br />(g) Applications for access to county or state roads shall be submitted <br />to the appropriate office of the county or state department of <br />Page 7 of 34 <br />
