CASSELBERRY CITY COMMISSION Minutes of Judy 8, 2013 — Regular Meeting Page 3 of 15 BA #13 -046 Explanation of Request: This Budget Amendment allocates additional funding to cover the costs associated with construction of the Casselberry Golf Course Pump Station Overhaul Project (PW 1202). Chinchor Electric, Inc. submitted the lowest responsible bid, in an amount not to exceed $69,907, to perform this work. The initial funding for this project ($65,000) was captured in the FY 2012 -2013 rollover. This budget amendment will provide sufficient funding for the City to begin construction. B. Change Order #4 with CFE Corporation for the North Winter Park Drive — Phase II Project and Related Budget Amendment #13 -056 Introduction: This item provides for notification to the City Commission of Change Order No. 4 to the agreement with Central Florida Environmental Corporation (CFE) in the amount of $22,03 5.70 for the North Winter Park Drive — Phase II Project (Proj ect Number PW 1003) which was approved by the City Manager on June 24, 2013. Consideration of related Budget Amendment #13 -056 is also requested. Background: At its October 25, 2010 meeting, the City Commission approved a Local Agency Program (LAP) agreement with the Florida Department of Transportation (FDOT) for the North Winter Park Drive — Phase H Project via Resolution 10 -2194. On February 14, 2011, the City Commission awarded a contract to CFE, which was determined to be the lowest, responsive, and responsible bidder at a total bid amount of $2,250,749.97. Since that time, three change orders have been approved by the City Commission as follows: June 27, 2011 - Change Order No. 1 - $134,049.20 New Total: $2,384,799.17 April 9, 2012 - Change Order No. 2 - ($154,698.90) New Total: $2,230,100.27 April 9, 2012 - Change Order No. 3 - $ 253293.20 New Total: $2,255,393.47 On May 14, 2012 via Resolution 12 -2376, the City Commission authorized the City Manager to execute Change Orders under $25,000.00 related to the North Winter Park Drive — Phase H Project and to place the Change Order on the Consent Agenda for Commission information. Budget Impact: The total costs associated with Change Order No. 4 are a net addition by $22,035.70. The allocation of costs related to this budget amendment is shown below:
