CASSELBERRY CITY COMMISSION <br />Minutes of Judy 8, 2013 — Regular Meeting <br />Page 3 of 15 <br />BA #13 -046 <br />Explanation of Request: This Budget Amendment allocates additional funding to cover the costs associated <br />with construction of the Casselberry Golf Course Pump Station Overhaul Project (PW 1202). Chinchor <br />Electric, Inc. submitted the lowest responsible bid, in an amount not to exceed $69,907, to perform this work. <br />The initial funding for this project ($65,000) was captured in the FY 2012 -2013 rollover. This budget <br />amendment will provide sufficient funding for the City to begin construction. <br />Account Number Account Description Amount <br />Increase: <br />401 - 0000 - 389.90 -00 Other Non-Operating Source $4,907 <br />401- 0753 - 535.63 -24 Irnprovements -Other $4,907 <br />Recommendation: The City Manager and the Public Works Director recommended approval of Resolution <br />13 -2501 and related Budget Amendment #13 -046. <br />B. Change Order #4 with CFE Corporation for the North Winter Park Drive — Phase II <br />Project and Related Budget Amendment #13 -056 <br />Introduction: This item provides for notification to the City Commission of Change Order No. 4 to the <br />agreement with Central Florida Environmental Corporation (CFE) in the amount of $22,03 5.70 for the North <br />Winter Park Drive — Phase II Project (Proj ect Number PW 1003) which was approved by the City Manager on <br />June 24, 2013. Consideration of related Budget Amendment #13 -056 is also requested. <br />Background: At its October 25, 2010 meeting, the City Commission approved a Local Agency Program <br />(LAP) agreement with the Florida Department of Transportation (FDOT) for the North Winter Park Drive — <br />Phase H Project via Resolution 10 -2194. On February 14, 2011, the City Commission awarded a contract to <br />CFE, which was determined to be the lowest, responsive, and responsible bidder at a total bid amount of <br />$2,250,749.97. Since that time, three change orders have been approved by the City Commission as follows: <br />June 27, 2011 - <br />Change Order No. 1 - <br />$134,049.20 <br />New Total: $2,384,799.17 <br />April 9, 2012 - <br />Change Order No. 2 - <br />($154,698.90) <br />New Total: $2,230,100.27 <br />April 9, 2012 - <br />Change Order No. 3 - <br />$ 253293.20 <br />New Total: $2,255,393.47 <br />On May 14, 2012 via Resolution 12 -2376, the City Commission authorized the City Manager to execute <br />Change Orders under $25,000.00 related to the North Winter Park Drive — Phase H Project and to place the <br />Change Order on the Consent Agenda for Commission information. <br />Budget Impact: The total costs associated with Change Order No. 4 are a net addition by $22,035.70. The <br />allocation of costs related to this budget amendment is shown below: <br />
