<br />4. SJRWMD Permit <br /> <br />This is an emergency maintenance of the banks of the canal due to erosion and <br />storm activity of the summer of 2008. <br /> <br />a. We will attend a preliminary meeting with the City of Casselberry. This <br />meeting will be used to explain and justify the project to SJRWMD for the <br />issuance of an expedited construction permit. <br /> <br />b. We will assist in the preparation of permit application forms and back-up data <br />in an effort to obtain approval of the project by SJRWMD. The permit <br />application fee shall be paid by the applicant directly to SJRWMD. <br /> <br />5. Construction Administration <br /> <br />Inspections and day to day coordination of the project shall be performed by the <br />City. This assumes concurrent construction at the Queens Mirror site and that both <br />projects can be inspected simultaneously. <br /> <br />a. CPH will attend a pre-construction meeting organized by the City. <br /> <br />b. CPH will provide one hour of inspector time per construction day of <br />inspection oversight services to supplement the activities of the City of <br />Casselberry. We anticipate performing this work in conjunction with the <br />simultaneous construction of the other sheet piling project. Length of <br />construction is estimated as 21 calendar days (15 working days). We will . <br />provide 15 hours for this project of inspector time under this proposal. <br /> <br />b. CPH, along with the attendance of representatives of the City, shall perform <br />a substantial completion inspection and develop a punch list. <br /> <br />c. The City shall perform the final completion inspection and verify that the <br />punch list items have been completed. The City shall provide written <br />verification to CPH that the items have been completed to their satisfaction. <br /> <br />d. Based on the inspections described above, CPH will prepare the Certificate <br />of Completion for SJRWMD. <br /> <br />A manpower estimate with associated costs is attached as Exhibit 2. <br />
