Cristina Viv6, Director of Personnel2~%~

SUBJECT: Proposed changes to the allocation of contributions to
the health insurance benefit

This is a request to submit the following employee health
insurance benefit changes to the City Commission:

1. For employees choosing the H~O Select, waive the $25
per month contribution; and
2. For employees choosing the HMO Select dependent
coverage, increase the City's contribution from $60 per
month to $75 per month.

There are currently two employees enrolled in the HMO Select.
Both employees are carrying dependent coverage. The cost of
dependent coverage has been an issue of concern consistently
raised by employees.

h'MO Select coverage has the lowest premiums cost of the three
health insurance benefit options offered by the City. The cost
of HMO Select single coverage is $317.41. The cost of the
standard HMO single coverage is $341.32. This represents for
the City a $23.91 savings over the standard HMO if an employee
chooses the HI~O Select single coverage.

As an inducement to move employees to the HMO Select and provide
assistance to employees with dependent coverage there are two
changes in contribution recommended. The savings realized by
the City if the employee chooses HI~O Select single coverage may
be passed on to employees by waiving the current $25 monthly
employee contribution. In addition, we propose increasing the
dependent contribution for employees who choose HMO Select
dependent coverage from $60 to $75 per month. The total
proposed assistance to employees with dependent insurance who

/Continued.
