EXHIBIT "A" <br />Wireless Infrastructure Provider shall mean a person who has been certificated under chapter 364 to <br />provide telecommunications service or under chapter 610 to provide cable or video services in this state <br />or that person's affiliate, to provide-teleEemmunisatiens se;Y;Ge iR the state and who builds or installs <br />wireless communication transmission equipment, wireless communication facilities, or wireless support <br />structures but is not a wireless services provider. <br />Wireless Provider shall mean a wireless infrastructure provider or a wireless services provider. <br />Wireless Services shall mean any services provided using licensed or unlicensed spectrum, whether <br />at a fixed location or mobile, using wireless communication facilities. <br />Wireless Services Provider shall mean a person who provides wireless services. <br />Wireless Support Structure shall mean a freestanding structure, such as a monopole, a guyed or <br />self-supporting tower, or another existing or proposed structure designed to support or sable capable of <br />supporting wireless communication facilities. The term does not include a utility pole pedestal or other <br />support structure for ground-based eauipment not mounted on a utility pole and_less than 5 feet in heiaht. <br />Wrap shall mean an aesthetic covering depicting scenic imagery, such as vegetation, which blends <br />with the surrounding area. <br />Sec. 68-154. - General prohibitions. <br />(a) Wireless communication facilities, other than small wireless facilities and micro wireless facilities, <br />are prohibited within the public rights-of-way. <br />(b) Wireless communication facilities are prohibited on arms used to support or mount traffic control <br />signals and warning signals and on arms attached to utility poles. <br />c) Guyed and lattice cellular towers are prohibited in the Public Riahts-of-Way. <br />Sec. 68-155. - Registration for placing or maintaining communications facilities in public rights-of-way. <br />(a) Applicability. All persons, including, but not limited to, a communications services provider, pass- <br />through provider, or wireless infrastructure provider, that desires to place or maintain a <br />communications facility in the public rights-of-way shall first register with the City in accordance with <br />this article before being eligible to receive a permit. Subject to the terms and conditions prescribed in <br />this article, and issuance of a permit, a registrant may place or maintain such facilities in public <br />rights-of-way. <br />(b) Application Requirements. Every communications services provider that desires to place or maintain <br />a Communications Facility in Public Rights -of -Way in the Citv shall file a single registration with the <br />City, on a form appreyed by the riff ER neer r designeewhich shall, at MiRiMUFR, include the <br />following information: <br />(1) Name of the r is r nt agplisaRt under which it will transact business in the City and, if <br />different, in the State of Florida; and <br />(2) Name, address and telephone number of a contact person for the registrant: 19 <br />ReRG, the <br />renisteFe � Rt 'n Florida; and <br />(3) The number of the registrant's current certificate of authorization issued by the Florida Public <br />Service Commission the Federal Communications Commission or the Department of State <br />
