1/23/2017 <br />STA TFRECOADIEVD.4TIONS. <br />Staff recommends the Planning and Zoning Commission approve an-Acasion request for SPR 14-13, <br />the Breereway at lake Marie Assisted Living Facility proposed at 1316, 1324 and 1331 Lake Drive. Staff <br />mcumdation is based on the original smffmpott dated March 9, 2016 (Eebibit C), the findings of fan <br />and conclusions of this staff report dared January 11, 2017, and the following conditions of apprord that <br />remain from the March 16, 2016 Planning and Zoning Commission appro,nl letter (Exhibit B): <br />1. All utility lines shall be placed underground (Section 3-10.2 (4). <br />2. All Public Works issues must be addressed prior to issuance of a site construction permit. <br />3. All required local, regional, state and federal pemrits shall be obtained before commencement of <br />development. <br />4. The applicant shall provide a photometric plan prior to issuance of a Certificate of Occupancy. This <br />shall cow rtn to the ULDR requa naccas for the street lighting. <br />5. The property shall pass landscaping, inigstion, and photometric/lighting inspections prior to <br />ompledon fthe prjen or issuance of a Certificate of Occupancy. <br />6. Payment into the Tree Bank (if found necessary) must be made prior to issuance of a Certificate of <br />Occupancy. <br />7. The site plan appro-I shall be sand for one year. An exaension request may be submitted prior to <br />the expiration fthis approval. <br />8. All of the above conditions shall be fully and faithfully executed or the site plan shall become null and <br />void. <br />
