This request is based —the site plan dated February 29, 2016, <br />the findings off et and conclusions of the information provided in the staffrepmt dated March 9, 2016, <br />and subject to the following condid-ce <br />Conditions ofAppoied: <br />1. All utility lines shall be placed underground (Section 3-10.2 (4). <br />2. All Public'W-ks issues mast be addressed prior to issuance of a site construction permit. <br />3. All required local, regional, state and federal permits shall be obtained before commencement <br />of development. <br />4. The applicant sball provide a pllommetricplan prior to issuance 'fa Certificate of Occupancy. <br />TM, <br />'ball conform to the ULDR requirements for the street lighting. <br />5. The prop'M shall pass landscaping, irrigation, and pbotometriolightingimpections prior to <br />ompletion of die project or issuance of a Certificate of Occupancy. <br />6. Palanent into the Tree Bank (if found necessary) most be made prior to issuance of a <br />Certificate of Occupancy. <br />7. The site plan approval shall be valid for one year. An -tension request maybe submitted <br />prior m the e.Pimtion of this approval. <br />8. All of the above conditions shalt be fully and fsiddidly executed or the site plan shall become <br />null and void. <br />3/22/2016 <br />RE111ELV <br />• Received Conditional Use approval to operate a Senior Living Facility from the Planning & <br />Zoning Commission on December 9,2015 (CCT 15-17) <br />• Meets all site requirements. <br />Providing twenty two (22) total off-street parking spaces, including two (2) ADA spaces. <br />Impervious Surface/Open space 65%135% required, providing 39,6/61% <br />(impervious/open space). <br />• Screening. The applicant will provide a well landscaped buffer on both die north and <br />south sides of the proposed development the buffer will consist of canopy trees, <br />understory trees, and shrubs. T11e buffer ranges from 10 to 20 feet <br />• Landscaping Tlhe applicant has provided the required number of trees per die City's <br />landscape requirements and has mitigated on-site for all trees being removed. <br />
