RESOLUTION 15 -2814 <br />A RESOLUTION OF THE CITY COMMISSION OF THE CITY OF <br />CASSELBERRY, FLORIDA, APPROVING AND AUTHORIZING THE <br />EXECUTION OF CHANGE ORDER NO.3 TO THE STANDARD <br />CONSULTANT AGREEMENT FOR PROFESSIONAL SERVICES FOR <br />BROWNFIELD CONTAMINATION MITIGATION WITH APPLIED <br />HYDROGEOLOGIC SOLUTIONS, LLC TO PROVIDE ADDITIONAL <br />CONSULTATION, CONSTRUCTION, MONITORING AND PERMITTING <br />SERVICES RELATED TO THE LAKE CONCORD PARK PROJECT; <br />PROVIDING FOR CONFLICTS, SEVERABILITY, AND EFFECTIVE <br />DATE. <br />WHEREAS, the City of Casselberry has planned for and implemented the Lake Concord <br />Park development south of City Hall on Triplet Lake Drive; and <br />WHEREAS, in order to help ensure construction is compliant with the required <br />mitigation of the brownfield portion of the property, the City's construction management <br />company, Wharton Smith, has requested the services of Applied Hydrogeologic Solutions, LLC, <br />(AHS) for consultation, construction, and permitting services during the construction phase; and <br />WHEREAS, on September 22, 2014, the City entered into a Standard Consultant <br />Agreement with AHS for professional services for brownfield mitigation of the former Shell <br />Station site at the corner of U.S. Hwy. 17 -92 and Triplet Lake Drive; and <br />WHEREAS, on February 9, 2015 the City entered into Change Order No. 1 with AHS <br />under aforesaid Standard Consultant Agreement to provide additional construction services for <br />the Shell Station site; and <br />WHEREAS, on May 11, 2015 the City entered into Change Order No. 2 with AHS <br />under aforesaid Standard Consultant Agreement to provide additional monitoring services for the <br />Shell Station site; and <br />WHEREAS, Amendment No. 1 was executed on October 22, 2015 to extend the AHS <br />contract to September 22, 2016; and <br />WHEREAS, due to various issues with the construction phase of the Lake Concord Park <br />project, the work effort required to assist the construction phase requires a change order to the <br />contract that will exceed AHS's original scope; and <br />WHEREAS, AHS provided a proposal in the amount of approximately $53,800 to <br />provide additional services for this project, which has been incorporated into the proposed <br />Change Order No. 3 to the Standard Consultant Agreement; and <br />WHEREAS, staff has reviewed the proposed Change Order No. 3 to the AHS Standard <br />Consultant Agreement and finds it sufficient to meet the needs of the City. <br />
