2) Said application shall contain <br />(a) Name and address of applicant if indivi- <br />dual, and if a firm, partnership, association or other group, the <br />name and address of every member thereof and in.the case of a <br />corporation, the name and address thereof and of its officers. <br />(b) Location of center. <br />(c) Names of all persons responsible for man- <br />agement, supervision or operation of center. <br />(d) The maximum number of children to be <br />enrolled., ages of children and hours of care. <br />(e) Annual Renewal of Permit - Any owner or operator of <br />a center permitted under this section shall on or before September <br />1st of each year make application for renewal of his license on <br />forms to be <br />furnished by the Town Clerk. <br />Said application <br />shall be <br />accompanied <br />by a permit renewal fee of Two <br />Dollars ($2.00). <br />Upon <br />receipt of such application properly filled in and executed., the <br />board shall issue a license to applicant authorizing the continua- <br />tion of the operation of said center for one (1) year. Said <br />application for renewal of permit shall be denied should applicant <br />fail to meet defined standards set up by the Commission. Within <br />ten (10) days from date of any denial an application for hearing <br />0 <br />must be submitted to the Commission; hearing will follow instanter. <br />(f) Inspections - Every center conducted hereunder shall <br />be subject to inspection by a duly authorized agent of the Commis- <br />sion at all reasonable times during hours of operation. <br />(g) Standards - No more than twenty-five (25%) per cent <br />of the first floor area of any main building may be devoted to use <br />as a center; further that there shall be no more than ten (10) <br />-3- <br />
