ATTACHMENT "A" <br />INSURANCE COVERAGE <br />1. Insurance Requirements: the Consultant shall obtain and furnish to the City of Casselberry, <br />prior to the contract being effective, Certificates of Insurance approved by the City with the <br />minimum coverages as listed below. All insurance policies shall be with insurers with an <br />acceptable rating, registered and licensed to do business in the State of Florida. <br />2. The Consultant shall name the City as an additional named insured with the following <br />minimum coverages. <br />a. Workers Compensation shall be maintained for all employees engaged in the work <br />under this BID, RFQ or RFP in accordance with the laws of the State of Florida. <br />Employers Liability Insurance shall be maintained by the selected firm or individual at <br />limits not less than the following: <br />$100,000 Each Accident <br />$100,000 Disease Each Employee <br />$500,000 Disease Aggregate <br />b. Comprehensive General Liability Insurance shall be maintained with limits not <br />less than the following: <br />$1,000,000 Bodily Injury & Property Damage -Each Occurrence <br />$1,000,000 Personal & Advertising Injury -Each Occurrence <br />$2,000,000 General Aggregate <br />$2,000,000 Products/Completed Operations Aggregates Limit <br />$5,000 Medical Payments <br />$100,000 Fire Damage Legal Liability <br />Coverage shall include Contractual Liability and Independent Contractors <br />Liability. <br />c. Automobile Liability Insurance shall be maintained with a combined single limit of <br />not less than $1,000,000 Bodily Injury and Property Damage in accordance with the <br />laws of the State of Florida, as to the Ownership, maintenance and use of all owned, <br />non-owned, leased or hired vehicles. <br />d. All insurance minimum coverage's extend to any sub-consultant and the General <br />Contractor is responsible for all sub-consultants. <br />All polices are to be endorsed to include the City as insured. In the cancellation <br />clause, the word endeavor shall be excluded and the number thirty (30) inserted in <br />the blank space provided before the word days-prior notice. All consultant polices <br />are to be considered primary to City coverage and shall not contain co-insurance <br />provisions. <br />Page 1 of 1-Attachment "A" <br />
