<br />V. SPECIFIC RESPONSIBILITIES OF THE PARTICIPANT <br /> <br />It shall be the responsibility of the Participant(s) assigned through this Agreement to: <br /> <br />A. Comply with the policies and procedures of the City and the City's Fire <br />Department. <br /> <br />B. Provide the necessary and appropriate uniform while on duty In the Fire <br />Department. <br /> <br />C. Obtain prior written approval of both parties to this Agreement before <br />publishing any material related to the learning experience provided under this <br />Agreement. <br /> <br />D. Sign a "Hold Harmless and Release Agreement" with the Fire Department prior <br />to commencing his/her experience within the Fire Department, a copy of which is <br />attached to this Agreement as Exhibit "A" and made a part hereof. <br /> <br />E. At all times, must wear the appropriate badge on every clinical, and comply <br />in all aspects with the student requirements set forth in the requirements <br />sheets provided by the Agency. <br /> <br />F. Designate an EMS program preceptor to be available to the Fire Department and <br />Participant by phone for any needs during their clinical learning experience. <br /> <br />VI. REQUEST FOR WITHDRAWAL OF PARTICIPANT <br /> <br />The Fire Department shall reserve the right to request the Agency to withdraw any <br />Participant from its facilities at any time for any reason in the Fire Department's sole discretion. <br /> <br />VII. MODIFICATION OF AGREEMENT <br /> <br />Modification of this Agreement may be made by mutual consent of both parties, in writing, and <br />attached to this Agreement and shall include the date and the signatures of parties agreeing to the <br />modification. <br /> <br />VIII. COPIES OF AGREEMENT <br /> <br />Copies of this signed Agreement and the Hold Harmless Agreement shall be placed on file and <br />be available at the corporate office of the Agency and in the offices of the Fire Department. <br /> <br />3 <br />
